About the position
As a Licensed Practical Nurse (LPN) at Interim Healthcare HomeCare, you will play a crucial role in providing compassionate care to patients in their homes. This position is designed for those who are dedicated to enhancing the quality of life for individuals in need of medical assistance. You will be responsible for delivering a full range of patient services that prioritize comfort and dignity. The role involves working overnight hours from 7 PM to 7 AM in Jamestown, PA, allowing for a unique opportunity to make a significant impact during the night shift when patients often require the most support. In this position, you will provide in-home direct patient care according to the prescribed plan of care from healthcare providers, while adhering to the nursing scope of practice. Your responsibilities will include closely monitoring patient progress and recovery, assessing and documenting patient conditions, and reporting any changes as necessary. You will administer medications, including insulin and IV fluids, ensuring thorough documentation of all treatments. Additionally, you will inspect and care for wounds, change dressings, and assist patients with personal hygiene needs. Collaboration is key in this role, as you will work alongside providers, aides, and allied health professionals to ensure exceptional patient outcomes. Your ability to assess patients for further injuries, infections, or bedsores and deliver appropriate treatments will be vital in maintaining the health and well-being of those in your care. Responsibilities
? Provide in-home direct patient care according to the provider's prescribed plan of care and nursing scope of practice.

? Closely monitor patient progress and recovery.

? Assess and document patient conditions, reporting changes as needed (vitals, blood/stool/urine, glucose, etc.).

? Administer medication, insulin, and IV fluids, documenting thoroughly.

Requirements
? Current unrestricted license to practice as a Licensed Practical Nurse (LPN) in the state associated with this position.

? Current CPR/AED/BLS/First Aid certification.

? Reliable transportation to/from care sites and/or work locations.
Nice-to-haves
? Practical trach and/or ventilator experience preferred, not required.
