Key Responsibilities

? Employ motivational interviewing techniques to educate and engage patients while managing follow-up care.
? Maintain and regularly update the patient registry.
? Accurately document care plans and clinical notes.
? Actively contribute to interdisciplinary care team meetings to ensure continuity of care and address patient needs.

Qualifications

Compensation And Benefits
